Patient rights:

Patient responsibilities:
- To attend appointments and be on time
- To ensure cell phones are off during appointments
- To treat others with courtesy and respect
- To come prepared (e.g., bring pill bottles for refills)
- To be an active participant in their care
- To follow the treatment plan
- To participate in protective measures (e.g., to help stop the spread of disease)
