Reminder! Please submit your Vaccination Attestation/Exemption Form
On August 31, UPEI updated its process for the UPEI Mandatory COVID-19 Vaccination Policy and issued a message to students, faculty, and staff.
All members of the University community (faculty, staff, and students) who are going to be on campus from September to December were asked to fill out the Attestation and Exemption Form at go.upei.ca/vaccine, regardless of their vaccination status. If you have already filled out the form, thank you, but if you haven’t, please do so as soon as possible.
In the form, you are asked to declare that you are either:
- fully vaccinated with a Health Canada-approved vaccine, and then you can upload an image or file (up to 20 MB) of your proof of vaccination.
- partially vaccinated with a Health Canada-approved vaccine, and then you will be asked to indicate when you expect to receive your second dose.
- not vaccinated with a Health Canada-approved vaccine, and then you can indicate when you expect to receive your vaccine, or that you require an exemption for medical, religious, or cultural reasons.
Here again is the link to the online Attestation and Exemption form that everyone must fill out: go.upei.ca/vaccine
To book an appointment for a COVID-19 vaccination, call (844) 975-3303 or visit PEI government’s online booking tool: https://pe.skipthewaitingroom.com/walk-in-clinic-provider/covid-vaccination-clinic
The UPEI Health and Wellness Centre will begin weekly COVID-19 testing soon, which is required for those who are not fully vaccinated. Please watch “Campus Notices” (also found in the my.UPEI.ca portal) to learn how to obtain testing. Those who are not fully vaccinated must also wear a mask at all times.
Regardless of a person's vaccination status, UPEI has also mandated the wearing of a mask in indoor spaces on campus as physical distancing of 2 metres/6 feet will be difficult to maintain in most settings: https://www.upei.ca/communications/news/2021/08/upei-re-introduce-mandatory-masking-effective-september-8-2021
